Appointments
You can request an appointment by calling our office at 574-223-9525 during normal business hours.   In the event you need to contact us outside of normal business hours, you may leave a voice mail message.  We will return your call within 24 hours.   Patients can be self-referred and/or referred by their physician based upon their insurance coverage.

Medication Requests
All medication requests should be called to the office between 8:00 a.m. and 3:00 p.m.  Your request will be called to the pharmacy of your choice within 24 hours.  When calling, please provide the following information:  your name and phone number, name of medication to be refilled, dosage, and name and phone number of the pharmacy for the refill.  Please allow us adequate time to refill your prescription.

Parking
Parking on the street is available free of charge. If you require handicap access, please call our office prior to your arrival. Our staff will be ready to assist you when you arrive.
